I agree, it's hard to know beforehand who should or need to be in a meeting or decision. I believe an open and transparent approach will help out making sure the right people receive the invitation or at least the notes so they can share their input. Then again, if crucial information comes up afterwards you have to be adaptable and make changes. With an inclusive environment I think we might get less of hallway decisions than in my experience we have in a not so inclusive workplace.
